    Workshops and roundtables are limited and will be filled on a first-received basis 
    of the completed registration form and complete registration fee. 
 
Conference Registration 

Please enclose payment in full with your registration. The registration fee must be paid by all attendees. Partial payments cannot be accepted. Make checks or money orders payable to the American Society of indexers and mail to P.O. Box 39366, Phoenix, AZ 85069-9366. Credit card payments will also be accepted. Please include your card number, expiration date and signature.

Refund Policy 

Requests for refunds must be made in writing to Bonnie Parks-Davis, Registrar. All refunds are subject to a $25 processing fee. Full refunds will be issued for requests postmarked prior to April 1. A 50% refund will be issued for requests postmarked between April 1 and May 15. Requests postmarked after May 15 cannot be honored. Refund checks will be issued within 6 weeks after the end of the conference. 

Change Policy 

Requests for changes must be made in writing to Bonnie Parks-Davis. No phone calls will be accepted. All requests for changes are subject to a $10 handling fee.

Hotel reservations must be made directly with the hotel, not as part of your conference registration. The conference rate is $99 plus 11% tax. This rate is guaranteed through May 11, 1999. Reservations made after this date are not guaranteed the convention rate. Hotel reservations may be made by calling the reservation department directly at 317-631-2221 or 1-800-2CROWNE World Wide Reservations. All you need to say is that you would like to make a reservation for the American Society of Indexers (ASI) meeting and give your dates of stay. Also available at a higher rate are authentic Pullman Train Car Rooms and Crowne Plaza Level rooms. 
 
NOTE: The conference hotel is sold out for Wednesday, June 9. We still have space available on Thursday, Friday, and Saturday nights at the $99 rate. It is being held through May11. 

The Crowne Plaza is referring people to the Holiday Inn North at 317-872-9790. This is the nearest Holiday Inn. It is possible that some transportation can be arranged if enough people are staying there.
